Forms and Documentation for New House Staff

The following forms are required for new interns and residents prior to the start of their training.  Send forms to the Department of Medical Education unless otherwise specified.

• Document checklist for new residents and interns
• NPI (National Provider Identification) number instructions
• I-9 Form (Instructions:  Complete this form along with copies of acceptable documents to verify citizenship and employability status.  For many individuals, this will consist of photocopies of your driver's license and your Social Security Card.  For the detailed booklet regarding this form, click here)
• New House Staff Basic Information Form (contact and other information)

2009 Annual Safety and Information Update ("Safety Fair") Documents  All hospital personnel need to complete annual training in safety, quality, and other issues.  House staff may fulfill the requirement by attending a Safety Fair session in the Auditorium.  See posted schedules for dates and times.  The alternative is to view the presentation and complete the online post-test.   Note:  New Safety Fair documentation will appear on this site in Fall 2010.
